Description
Overview of the medicine
Amoxicillin is a broad-spectrum penicillin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
General adult dosage (based on drug, not strictly suspension): 250-500 mg every 8 hours, or 500-875 mg every 12 hours depending on infection severity. For suspension, typically 10-20 ml (250-500 mg) every 8 hours.
Elderly
Similar to adults, but renal function should be considered and dosage adjusted if necessary.
Renal_impairment
Dose adjustment required; e.g., for creatinine clearance < 30 ml/min, extend dosing interval or reduce dose.
How to Take
Administer orally, with or without food. Shake the bottle well before each use. Use an accurate measuring spoon or oral syringe to measure the dose.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.
Pharmacokinetics
Onset
30-60 minutes
Excretion
Primarily excreted unchanged in urine via renal tubular secretion and glomerular filtration
Half life
1-1.5 hours in adults with normal renal function
Absorption
Rapidly and well absorbed after oral administration, bioavailability 70-90%. Peak plasma concentrations within 1-2 hours.
Metabolism
Partially metabolized (10-25%) to inactive penicilloic acid
Side Effects
Contraindications
- Hypersensitivity to amoxicillin, any other penicillin, or any component of the formulation.
- History of severe hypersensitivity reaction to another beta-lactam agent (e.g., cephalosporins, carbapenems).
Drug Interactions
Probenecid
Increases and prolongs amoxicillin blood levels by reducing renal tubular secretion.
Allopurinol
Increased risk of skin rash, especially in patients with hyperuricemia.
Methotrexate
May increase methotrexate toxicity due to reduced renal excretion.
Oral Contraceptives
May reduce the efficacy of oral contraceptives; advise use of alternative birth control methods.
Warfarin and other oral anticoagulants
May increase the anticoagulant effect (monitor INR/PT closely).
Storage
Store dry powder at room temperature (below 30°C), protected from moisture. Store reconstituted suspension in a refrigerator (2-8°C), do not freeze.
Overdose
Overdose symptoms may include gastrointestinal symptoms (nausea, vomiting, diarrhea) and fluid/electrolyte imbalance. Renal impairment may lead to crystalluria. Treatment is supportive; hemodialysis can remove amoxicillin.
Pregnancy & Lactation
Pregnancy: Category B. Generally considered safe; use only if clearly needed. Lactation: Excreted in breast milk in small amounts; generally considered safe, but monitor for diarrhea or candidiasis in infants.

YesClinical Trials
Amoxicillin has been extensively studied in numerous clinical trials supporting its efficacy and safety for a wide range of bacterial infections across various patient populations.
Lab Monitoring
- Periodic assessment of organ system function, including renal, hepatic, and hematopoietic function during prolonged therapy.
- Culture and susceptibility testing prior to and during treatment to confirm microbial sensitivity.
Doctor Notes
- Always inquire about penicillin allergy before prescribing amoxicillin.
- Monitor patients for signs of allergic reactions, especially during the first dose.
- Advise patients to stay well-hydrated to minimize the risk of crystalluria.
- Consider culture and sensitivity tests for persistent or recurrent infections.
Patient Guidelines
- Complete the full course of treatment, even if symptoms improve, to prevent antibiotic resistance.
- Shake the suspension bottle well before each use and measure the dose accurately.
- Discard any unused portion of the reconstituted suspension after the specified period (e.g., 7-14 days), even if it looks okay.
- Inform your doctor about any history of allergic reactions to antibiotics.
Missed Dose Advice
Take the missed dose as soon as you remember, unless it is almost time for your next scheduled dose. In that case, skip the missed dose and resume your regular dosing schedule. Do not double the dose to make up for a missed one.
Driving Precautions
Amoxicillin generally has no known effects on the ability to drive or operate machinery. However, if side effects like dizziness or confusion occur, avoid driving or operating machinery.
Lifestyle Advice
- Maintain good hygiene to prevent spread of infection.
- Stay well-hydrated throughout the course of treatment.
- Avoid alcohol during treatment, as it may exacerbate gastrointestinal side effects.
