Description
Overview of the medicine
Ambroxol is a mucolytic agent used to treat respiratory diseases associated with viscid mucus. It helps to thin and loosen mucus in the airways, making it easier to cough up.
Uses & Indications
Dosage
Adults
10 ml (30 mg ambroxol) 3 times daily. In severe cases, dosage may be increased to 20 ml (60 mg ambroxol) twice daily.
Elderly
Similar to adult dosage, but caution with comorbidities and reduced renal/hepatic function.
Renal_impairment
Dosage reduction (e.g., half the standard dose or extended intervals) may be necessary in severe renal impairment.
How to Take
Oral. Can be taken with or without food. Use a measuring spoon or cup for accurate dosage.
Mechanism of Action
Ambroxol acts as a mucolytic by depolymerizing mucopolysaccharides directly as well as through stimulation of lysosomal enzymes. It also enhances mucociliary clearance and stimulates surfactant production, reducing mucus viscosity and improving its transport.
Pharmacokinetics
Onset
Within 30 minutes.
Excretion
Mainly via the kidneys (approximately 90%) as metabolites, with about 10% excreted unchanged.
Half life
Approximately 10 hours for the parent compound, with active metabolites having a shorter half-life.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached after 1-2.5 hours.
Metabolism
Primarily in the liver via glucuronidation and some cleavage into dibromanthranilic acid and other metabolites.
Side Effects
Contraindications
- Hypersensitivity to ambroxol or any other component of the formulation.
- Rare hereditary conditions that may be incompatible with an excipient of the product (e.g., fructose intolerance if containing sorbitol).
- Gastric and duodenal ulcers (relative contraindication, use with caution).
Drug Interactions
Antitussives (Cough Suppressants)
Concomitant use is not recommended as it may lead to a build-up of secretions in the airways due to suppression of the cough reflex.
Antibiotics (e.g., Amoxicillin, Cefuroxime, Erythromycin, Doxycycline)
Ambroxol may increase the penetration of these antibiotics into bronchial secretions, potentially enhancing their efficacy.
Storage
Store below 30°C in a dry place. Protect from light and moisture. Keep out of reach and sight of children.
Overdose
Symptoms typically include restlessness, diarrhea, nausea, vomiting, and dyspepsia. Management involves symptomatic and supportive treatment. Gastric lavage and induced emesis are not usually indicated.
Pregnancy & Lactation
Should only be used during pregnancy, particularly during the first trimester, and lactation after careful risk-benefit assessment by a physician. Consult a doctor before use.

YesClinical Trials
Ambroxol has been extensively studied in various clinical trials for its efficacy and safety in respiratory conditions, demonstrating its role as a well-established mucolytic agent.
Lab Monitoring
- Generally not required for routine use.
Doctor Notes
- Emphasize the importance of adequate hydration to patients to maximize mucolytic effect.
- Advise against co-administration with antitussives to prevent mucus retention.
- Counsel patients on potential rare severe skin reactions and to seek immediate medical attention if they occur.
- Dosage adjustment may be needed in severe renal or hepatic impairment.
Patient Guidelines
- Complete the full course of treatment as prescribed by your doctor.
- Drink plenty of fluids (water, juices) to help thin mucus and aid expectoration.
- Do not use this medicine concurrently with cough suppressants, as it may cause mucus build-up.
- If you experience any severe skin reactions (e.g., rash with blistering, peeling), discontinue use and seek immediate medical attention.
Missed Dose Advice
Take the missed dose as soon as you remember, unless it is almost time for your next dose. Do not double the dose to catch up.
Driving Precautions
Ambroxol generally has no influence on the ability to drive or operate machinery. However, if you experience side effects like dizziness, avoid such activities.
Lifestyle Advice
- Avoid smoking, as it irritates the respiratory tract and exacerbates mucus production.
- Maintain good hydration by drinking adequate fluids.
- Practice good cough hygiene to prevent spread of infection.
