Description
Overview of the medicine
Antogin-ER 500 mg Tablet is an extended-release non-steroidal anti-inflammatory drug (NSAID) containing Naproxen Sodium. It is used for the relief of pain, inflammation, and stiffness associated with various musculoskeletal and rheumatic conditions.
Uses & Indications
Dosage
Adults
For chronic conditions such as osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis, the usual dose is 500 mg or 750 mg once daily. Maximum dose should not exceed 1000 mg/day. For acute pain, up to 1000 mg once daily for short periods.
Elderly
Lower doses may be required, and renal function should be closely monitored due to increased susceptibility to adverse effects.
Renal_impairment
Not recommended for patients with severe renal impairment (creatinine clearance < 30 mL/min). Dose reduction is advised for moderate renal impairment with close monitoring.
How to Take
Antogin-ER 500 mg Tablets should be swallowed whole with water. Do not crush, chew, or break the tablet. It can be taken with food or milk to reduce gastrointestinal upset.
Mechanism of Action
Naproxen Sodium works by inhibiting the activity of cyclooxygenase (COX) enzymes, specifically both COX-1 and COX-2. This inhibition leads to a reduction in the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever.
Pharmacokinetics
Onset
Onset of action for pain relief is typically delayed for extended-release formulations, usually within several hours.
Excretion
Approximately 95% of naproxen is excreted in the urine as unchanged drug and metabolites, with a small amount excreted in the feces.
Half life
The plasma elimination half-life of naproxen is approximately 12 to 17 hours, which is sustained due to the extended-release formulation.
Absorption
Naproxen Sodium is well and rapidly absorbed from the gastrointestinal tract. The extended-release formulation delays the peak plasma concentration compared to immediate-release forms.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes to 6-O-desmethylnaproxen, with minimal active metabolites.
Side Effects
Contraindications
- History of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs.
- Active peptic ulcer or gastrointestinal bleeding.
- Severe heart failure.
- Third trimester of pregnancy due to risk of premature closure of the fetal ductus arteriosus.
- Severe renal or hepatic impairment.
Drug Interactions
Lithium
Increased plasma lithium levels, leading to potential toxicity.
Methotrexate
Increased methotrexate toxicity.
SSRIs and SNRIs
Increased risk of gastrointestinal bleeding.
Diuretics and ACE inhibitors
Reduced antihypertensive effect and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, heartburn, indigestion, nausea, vomiting, epigastric pain, and rarely gastrointestinal bleeding. Management is symptomatic and supportive. Gastric lavage and/or activated charcoal may be considered within 1 hour of ingestion.
Pregnancy & Lactation
Pregnancy Category C in the first and second trimesters, and Category D in the third trimester (due to potential adverse effects on the fetus). Should be avoided during lactation as naproxen is excreted in breast milk.

Clinical Trials
Extensive clinical trials have established the efficacy and safety of naproxen for its approved indications. Studies specifically on extended-release formulations demonstrate sustained drug levels and prolonged pain relief.
Lab Monitoring
- Renal function tests (e.g., serum creatinine, BUN) should be monitored periodically, especially in elderly patients or those with pre-existing renal impairment.
- Liver function tests (e.g., ALT, AST) may be monitored, particularly during long-term therapy.
- Complete blood count (CBC) should be checked for signs of gastrointestinal bleeding or anemia.
Doctor Notes
- Assess cardiovascular risk factors and gastrointestinal history before initiating NSAID therapy, especially for long-term use.
- Monitor renal function, liver enzymes, and complete blood count periodically in patients on long-term Antogin-ER therapy.
- Prescribe the lowest effective dose for the shortest possible duration to minimize risks.
- Advise patients on symptoms of GI bleeding and cardiovascular events and instruct them to seek immediate medical attention if they occur.
Patient Guidelines
- Take Antogin-ER with food or milk to minimize stomach upset.
- Report any unusual bleeding, black or tarry stools, severe stomach pain, or swelling to your doctor immediately.
- Do not take other NSAIDs (including aspirin) concurrently without consulting your doctor.
- Inform your doctor about all other medications, supplements, and herbal products you are taking.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Antogin-ER may cause dizziness, drowsiness, or visual disturbances in some patients. Patients should be cautious when driving or operating machinery until they know how the medicine affects them.
Lifestyle Advice
- Avoid alcohol consumption, as it may increase the risk of gastrointestinal irritation and bleeding.
- Maintain a healthy diet and stay hydrated. Regular, moderate exercise may also help manage pain.
