Description
Overview of the medicine
Montelukast is a leukotriene receptor antagonist used for the prophylaxis and chronic treatment of asthma, and for the relief of symptoms of seasonal allergic rhinitis. It works by blocking leukotrienes, which are substances released in the body that cause inflammation and narrowing of airways.
Uses & Indications
Dosage
Adults
Not typically recommended for adults as 5mg, as usual adult dose is 10mg. If prescribed 5mg, take once daily in the evening.
Elderly
No dosage adjustment necessary.
Children
Children (6-14 years): 5 mg once daily in the evening.
Renal_impairment
No dosage adjustment necessary.
How to Take
Place the flash tablet on the tongue and allow it to dissolve completely before swallowing. It can be taken with or without water. Do not chew the tablet. For asthma and allergic rhinitis, take once daily in the evening. For exercise-induced bronchoconstriction, take at least 2 hours before exercise.
Mechanism of Action
Montelukast selectively antagonizes the cysteinyl leukotriene 1 (CysLT1) receptor, preventing the binding of leukotrienes (LTC4, LTD4, LTE4). This inhibition reduces bronchoconstriction, inflammation, and mucus secretion, leading to improved airflow and reduced allergy symptoms.

Onset
Within 1 day for asthma/allergic rhinitis relief.
Excretion
Primarily via bile in feces (>86%), with very little (<0.2%) excreted in urine.
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration reached in 2-4 hours. Oral bioavailability is approximately 64%.
Metabolism
Extensively metabolized in the liver by CYP3A4, 2C9, and 2C8.
Side Effects
Contraindications
- Known hypersensitivity to montelukast or any component of the formulation.
Drug Interactions
Rifampin
May decrease montelukast plasma concentrations by approximately 40%.
Phenobarbital
May decrease montelukast plasma concentrations (AUC) by approximately 40%.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, seek emergency medical attention. Symptoms may include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive. It is not known whether montelukast is dialyzable by peritoneal dialysis or hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in pregnant women have not shown an increased risk of fetal abnormalities. Use during pregnancy only if clearly needed. Montelukast is excreted in breast milk in animal studies; caution should be exercised when administered to a nursing mother.
