Description
Overview of the medicine
Axlovir 150 mg Tablet is an antiviral medication used for the treatment of certain severe viral infections. It works by inhibiting the replication of the virus, thereby helping the immune system to clear the infection. It is prescribed for a limited duration.
Uses & Indications
Dosage
Adults
150 mg orally twice daily for 5-7 days, depending on the severity of infection.
Elderly
No specific dose adjustment is required for elderly patients unless renal impairment is present.
Renal_impairment
For creatinine clearance (CrCl) 30-50 mL/min: 150 mg once daily. For CrCl <30 mL/min: Not recommended or use with extreme caution.
How to Take
Take the tablet orally with a glass of water, with or without food. It is important to complete the full course of treatment as prescribed.
Mechanism of Action
Axlovir selectively inhibits the viral RNA-dependent RNA polymerase (RdRp), which is essential for viral replication. By binding to the active site of RdRp, it prevents the synthesis of new viral RNA strands, effectively halting viral proliferation.
Pharmacokinetics
Onset
Clinical improvement typically observed within 2-3 days of initiation of therapy.
Excretion
Excreted predominantly via urine (60-70%) as metabolites, with some fecal excretion (20-30%).
Half life
Elimination half-life is approximately 6-8 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 1-2 hours. Bioavailability is approximately 80%.
Metabolism
Primarily metabolized in the liver via CYP3A4 pathway to an active metabolite.
Side Effects
Contraindications
- Hypersensitivity to Axlovir or any components of the tablet.
- Severe hepatic impairment.
Drug Interactions
Warfarin
May increase anticoagulant effect; monitor INR closely.
CYP3A4 Inducers (e.g., Rifampin, Phenytoin)
May decrease Axlovir plasma levels, reducing efficacy.
CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir)
May increase Axlovir plasma levels, increasing risk of side effects.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote. Hemodialysis is unlikely to be effective due to high protein binding.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is not known if Axlovir is excreted in human milk; caution should be exercised when administered to a nursing mother.

Lab Monitoring
- Liver function tests (ALT, AST) before and during treatment.
- Renal function tests (Creatinine, BUN) periodically.
- Viral load measurements to assess treatment response.
Doctor Notes
- Emphasize adherence to the full treatment course.
- Monitor liver function tests, especially in patients with pre-existing hepatic conditions.
- Advise patients on potential drug interactions.
Patient Guidelines
- Complete the entire course of medication, even if symptoms improve.
- Do not share this medication with others.
- Report any severe or persistent side effects to your doctor immediately.
