Description
Overview of the medicine
Bendamax 25 mg Injection contains Bendamycin Hydrochloride, a broad-spectrum third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Usual dose: 25-50 mg every 8-12 hours, administered intramuscularly (IM) or intravenously (IV). For severe infections, up to 100 mg every 8 hours.
Elderly
No specific dose adjustment typically required, but monitor renal function. Max dose 75 mg every 12 hours.
Renal_impairment
Adjust dose based on creatinine clearance (CrCl). For CrCl 10-50 mL/min, administer 25 mg every 12 hours. For CrCl <10 mL/min, administer 25 mg every 24 hours.
How to Take
Bendamax 25 mg Injection can be administered intramuscularly (IM) or intravenously (IV). For IM use, dissolve in appropriate diluent and inject deep into a large muscle mass. For IV use, dilute in 50-100 mL of compatible IV fluid (e.g., Normal Saline, Dextrose 5%) and infuse over 30-60 minutes. Do not administer intrathecally.
Mechanism of Action
Bendamycin acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, preventing the cross-linking of peptidoglycan chains, which is essential for cell wall strength and integrity, leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Within 30 minutes (IV), 1-2 hours (IM)
Excretion
Mainly excreted by the kidneys through glomerular filtration and tubular secretion. Approximately 50-60% of the dose is excreted unchanged in urine within 24 hours.
Half life
Approximately 1.5-2 hours
Absorption
Rapid and complete absorption after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are achieved within 1-2 hours after IM injection.
Metabolism
Limited hepatic metabolism; primarily excreted as unchanged drug.
Side Effects
Contraindications
- Hypersensitivity to Bendamycin, other cephalosporins, or any component of the formulation.
- History of severe allergic reactions to penicillin antibiotics.
Drug Interactions
Probenecid
May increase and prolong Bendamycin serum concentrations.
Aminoglycosides
Increased risk of nephrotoxicity (kidney damage).
Loop Diuretics (e.g., Furosemide)
Increased risk of nephrotoxicity, especially with aminoglycosides.
Oral Anticoagulants (e.g., Warfarin)
May enhance anticoagulant effect, requiring monitoring of INR.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Do not freeze. Keep out of reach of children.
Overdose
In case of overdose, supportive and symptomatic treatment should be provided. Hemodialysis may be useful in enhancing elimination, especially in patients with impaired renal function.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Bendamycin is excreted in breast milk in small amounts; use with caution in breastfeeding mothers.
