Overview of the medicine
Bulex-75-mg syrup is a combination medication used to treat cough and congestion associated with various respiratory conditions. It contains a mucolytic and an expectorant.
Uses & Indications
Dosage
Adults
Adults: 10 ml (2 teaspoonfuls) 3 times daily.
Elderly
No specific dose adjustment is required, but use with caution in those with pre-existing conditions.
Children
Children 6-12 years: 5 ml (1 teaspoonful) 3 times daily; Children 2-6 years: 2.5 ml (½ teaspoonful) 3 times daily; Below 2 years: Consult a physician.
Renal_impairment
Use with caution in severe renal impairment; dose reduction may be necessary.
How to Take
To be taken orally, with or without food. Measure dose with an accurate measuring device.
Mechanism of Action
Bromhexine thins and loosens mucus by depolymerizing mucopolysaccharide fibers. Guaifenesin increases the volume and reduces the viscosity of secretions in the trachea and bronchi, thereby facilitating the removal of phlegm.

Bromhexine and its metabolites are primarily excreted via the kidneys. Guaifenesin and its metabolites are excreted via the kidneys.

Bromhexine: 15-20 hours (terminal); Guaifenesin: Approx. 1 hour
Absorption
Bromhexine is rapidly and almost completely absorbed from the GI tract. Guaifenesin is rapidly absorbed from the GI tract.
Metabolism
Bromhexine undergoes extensive first-pass metabolism in the liver. Guaifenesin undergoes rapid metabolism.
Side Effects
Contraindications
- Hypersensitivity to Bromhexine, Guaifenesin, or any component of the formulation
- Gastric ulcer (relative contraindication for Bromhexine due to potential gastric irritation)
Drug Interactions
Antibiotics
Bromhexine may increase penetration of certain antibiotics (e.g., amoxicillin, erythromycin) into bronchial secretions.
Antitussives
Concurrent use with antitussives (cough suppressants) is not recommended as it may lead to accumulation of secretions due to suppression of cough reflex.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, diarrhea, and gastric irritation. Treatment is symptomatic and supportive. Gastric lavage may be considered.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks. Caution advised during lactation as components may be excreted in breast milk.
