Description
Overview of the medicine
Calcibost-D 500 mg tablet is a combination supplement containing Calcium Carbonate and Vitamin D3. It is primarily used to maintain healthy bones and teeth, and for the prevention and treatment of calcium and Vitamin D deficiencies.
Uses & Indications
Dosage
Adults
Usually 1-2 tablets daily, or as directed by physician. May be taken in divided doses.
Elderly
Same as adult dosage, but caution with renal function. Consult physician.
Renal_impairment
Use with caution. Dosage adjustment may be required based on serum calcium and phosphate levels. Contraindicated in severe renal impairment.
How to Take
Oral administration. Tablets can be taken with or without food, but generally recommended to be taken with food for better absorption and to minimize gastrointestinal upset. Do not exceed the recommended dose.
Mechanism of Action
Calcium is an essential mineral required for bone formation, nerve impulse transmission, muscle contraction, and blood coagulation. Vitamin D3 (Cholecalciferol) helps regulate calcium and phosphate homeostasis, facilitating their absorption from the gut and deposition into bone. It also plays a role in immune function.
Pharmacokinetics
Onset
Therapeutic effects on bone density are gradual and require consistent intake over several weeks to months.
Excretion
Unabsorbed calcium is excreted in feces. Absorbed calcium is excreted primarily via kidneys. Vitamin D3 metabolites are excreted mainly in bile and feces, with a small amount in urine.
Half life
Elemental Calcium has a relatively short half-life in blood, but its biological half-life in bone is much longer. Vitamin D3 has a half-life of about 15-20 hours, but its active metabolite calcitriol has a longer half-life.
Absorption
Calcium absorption is variable and depends on factors like age, Vitamin D status, and dietary intake. Vitamin D3 is well absorbed from the gastrointestinal tract, primarily in the small intestine.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol), its active form.
Side Effects
Contraindications
- Hypersensitivity to Calcium Carbonate, Vitamin D3, or any component of the formulation
- Hypercalcemia (high levels of calcium in blood)
- Hypercalciuria (high levels of calcium in urine)
- Severe renal impairment
- Hypervitaminosis D
Drug Interactions
Digoxin
Hypercalcemia can increase the risk of digoxin toxicity. Monitor closely.
Levothyroxine
Calcium may impair absorption. Separate administration by at least 4 hours.
Bisphosphonates
Calcium can interfere with bisphosphonate absorption. Administer calcium at least 30 minutes after bisphosphonates.
Thiazide diuretics
May reduce calcium excretion, increasing the risk of hypercalcemia.
Tetracycline and Fluoroquinolone antibiotics
Calcium can reduce the absorption of these antibiotics. Administer at least 2 hours before or 4-6 hours after calcium supplements.
Storage
Store in a cool, dry place, away from direct light and moisture, below 30°C. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, thirst, polyuria, muscle weakness, confusion, kidney stones). In severe cases, cardiac arrhythmias may occur. Treatment involves discontinuing the supplement, hydration, and medical management to lower calcium levels.
Pregnancy & Lactation
Generally considered safe and often recommended during pregnancy and lactation to meet increased calcium and Vitamin D requirements. However, always consult a healthcare professional before use. Excessive doses should be avoided.

Clinical Trials
Numerous clinical trials support the efficacy of calcium and Vitamin D supplementation in improving bone mineral density, reducing fracture risk in at-risk populations, and treating deficiency states. Specific trials for Calcibost-D would be conducted by the manufacturer for regulatory approval.
Lab Monitoring
- Serum calcium levels (especially in patients with renal impairment or on long-term therapy)
- Serum Vitamin D levels (25-hydroxyvitamin D)
- Renal function (creatinine, eGFR) in at-risk patients
Doctor Notes
- Assess patient's dietary calcium and Vitamin D intake before prescribing.
- Consider underlying conditions (e.g., malabsorption, renal dysfunction) that may affect calcium/Vitamin D metabolism.
- Counsel patients on potential drug interactions, especially with antibiotics and thyroid hormones.
- Advise on lifestyle modifications for optimal bone health.
Patient Guidelines
- Take exactly as directed by your doctor or pharmacist.
- Do not exceed the recommended dose.
- Take with a full glass of water.
- Inform your doctor about all other medications you are taking, especially antibiotics, thyroid hormones, and diuretics.
- Maintain adequate fluid intake.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Calcibost-D is not expected to affect the ability to drive or operate machinery.
Lifestyle Advice
- Engage in regular weight-bearing exercise to support bone health.
- Maintain a balanced diet rich in calcium (e.g., dairy products, leafy greens) and Vitamin D (e.g., fatty fish, fortified foods).
- Moderate sun exposure can help the body produce Vitamin D.
