Cefixime is an oral third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections, including those of the respiratory tract, urinary tract, and ear infections.

Upper and lower respiratory tract infections (e.g., pharyngitis, tonsillitis, acute bronchitis)

Urinary tract infections (uncomplicated)

Dosage

Adults

Usual adult dose is 200 mg twice daily or 400 mg once daily, depending on the severity of infection. Treatment duration typically 7-14 days.

Elderly

No dosage adjustment needed for elderly patients unless there is significant renal impairment.

Renal_impairment

For creatinine clearance 21-60 mL/min, dose should be reduced to 300 mg daily. For creatinine clearance <20 mL/min, dose should be reduced to 200 mg daily.

How to Take

Take orally, with or without food. For best absorption, some suggest taking it with food to reduce gastrointestinal upset.

Mechanism of Action

Cefixime acts by inhibiting bacterial cell wall synthesis, leading to cell lysis and bacterial death. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, preventing the cross-linking of peptidoglycan chains which are crucial for bacterial cell wall strength and rigidity.
