Description
Overview of the medicine
Clindamycin is a lincosamide antibiotic used to treat various serious bacterial infections, including those of the respiratory tract, skin, soft tissue, bone, and gynecological infections. It is effective against a wide range of anaerobic bacteria and Gram-positive aerobic organisms.
Uses & Indications
Dosage
Adults
For serious infections: 150-300 mg every 6-8 hours. For more severe infections: 300-450 mg every 6-8 hours. Maximum 1.8 g/day orally.
Elderly
Similar to adult dosage; however, monitor renal/hepatic function and adjust as necessary.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. For severe renal impairment (CrCl < 30 mL/min) or anuria, dose reduction or increased dosing interval may be considered.
How to Take
Take orally with a full glass of water to prevent esophageal irritation. Can be taken with or without food.
Mechanism of Action
Clindamycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby preventing the translocation of peptide chains. This action is primarily bacteriostatic, but can be bactericidal at higher concentrations or against highly susceptible organisms.
Pharmacokinetics
Onset
Therapeutic effect usually observed within 1 hour.
Excretion
Approximately 10-20% excreted in urine as active drug and metabolites, 4% in feces, and the remainder via bile.
Half life
Approximately 2-3 hours in adults; can be prolonged in patients with severe renal or hepatic impairment.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract (approximately 90%). Peak plasma concentrations are reached within 45-60 minutes after oral administration.
Metabolism
Extensively metabolized in the liver by CYP3A4 into active (N-demethyl clindamycin) and inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to clindamycin, lincomycin, or any component of the formulation.
- History of antibiotic-associated colitis.
Drug Interactions
Erythromycin
Antagonism between clindamycin and erythromycin has been demonstrated in vitro; avoid concomitant use.
Kaolin-pectin
May decrease the absorption of clindamycin; separate administration by several hours.
Neuromuscular blocking agents
May enhance the action of neuromuscular blocking agents, leading to increased muscle weakness.
Warfarin and similar anticoagulants
May increase the anticoagulant effect, leading to increased bleeding risk; monitor INR closely.
Storage
Store below 30°C in a cool, dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
No specific antidote for clindamycin overdose. Management should be symptomatic and supportive, including maintaining airway, breathing, and circulation. Hemodialysis and peritoneal dialysis are not effective in removing clindamycin from the blood.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use in pregnant women should be only if clearly needed. Clindamycin is excreted in breast milk; therefore, caution should be exercised when administered to a nursing mother.
