Emonium 5 mg Injection is an antiemetic drug used to prevent and treat nausea and vomiting, particularly those associated with chemotherapy, radiotherapy, or surgery. It works by blocking serotonin, a natural substance in the body that can cause nausea and vomiting.
Uses & Indications
Dosage
Adults
For chemotherapy-induced nausea and vomiting (CINV): 5 mg or 8 mg IV infused over 15 minutes, 30 minutes prior to chemotherapy. Repeat doses may be given at 4 or 8 hour intervals. For post-operative nausea and vomiting (PONV): 5 mg IV administered immediately before induction of anesthesia or shortly after surgery.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required in patients with renal impairment.
How to Take
Emonium injection should be administered intravenously, usually as a slow injection over 2-5 minutes or diluted and infused over 15 minutes. It must be administered by a healthcare professional.
Mechanism of Action
Emonium selectively blocks 5-HT3 receptors located both peripherally on vagal nerve terminals and centrally in the chemoreceptor trigger zone (CTZ) of the area postrema. By blocking these receptors, it inhibits the initiation of the vomiting reflex caused by emetogenic stimuli such as chemotherapy-induced serotonin release.

Onset
Within 30 minutes of intravenous administration.
Excretion
About 60-70% excreted in urine as metabolites, with a small portion excreted unchanged. Approximately 5% excreted in feces.
Half life
Approximately 4-6 hours.
Absorption
Rapidly and completely absorbed following intravenous administration, reaching peak plasma concentrations within minutes.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes (CYP2D6, CYP1A2, CYP3A4) via hydroxylation, followed by glucuronide and sulfate conjugation.
Side Effects
Contraindications
- Known hypersensitivity to Emonium or any component of the injection.
- Concomitant use with apomorphine due to reported profound hypotension and loss of consciousness.
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol; also, cases of serotonin syndrome have been reported.
Apomorphine
Concomitant use with Emonium is contraindicated due to severe hypotension and loss of consciousness.
Drugs prolonging QT interval (e.g., antiarrhythmics, some antipsychotics)
Increased risk of QT interval prolongation and Torsade de Pointes. Caution advised.
Storage
Store below 30°C, protected from light. Do not freeze. Keep out of reach of children.
Overdose
There is no specific antidote for Emonium overdose. Management should be symptomatic and supportive, including monitoring of ECG for QT prolongation.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. It is unknown whether Emonium is excreted in human milk; caution should be exercised when administered to a nursing mother.
