Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for 4-8 weeks. For H. pylori eradication: 40 mg once daily for 10 days (in combination therapy). For Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted up to 120 mg twice daily.
Elderly
No specific dosage adjustment is generally needed based on age alone.
Renal_impairment
No dosage adjustment is typically required for patients with renal impairment.
How to Take
Take orally, swallow the tablet whole with water, at least one hour before a meal. Do not crush or chew the tablet.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that works by irreversibly blocking the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells, thereby inhibiting the final step of gastric acid secretion.

Excretion
Excreted primarily in the urine (about 80%) as metabolites, and to a lesser extent in the feces (about 20%).
Half life
Approximately 1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% after a single 40 mg dose, increasing to 89% with repeated once-daily administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and to a lesser extent by CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, other substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir (an antiretroviral drug).
Drug Interactions
Digoxin
Increased plasma levels of digoxin.
Warfarin
Increased INR and prothrombin time have been reported with concomitant use, requiring monitoring.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19.
Methotrexate
May increase methotrexate levels and/or its metabolite, especially with high doses.
Nelfinavir/Atazanavir
Significantly reduced plasma levels, leading to loss of antiviral efficacy.

Overdose
Experience with esomeprazole overdose is limited. Symptoms may include gastrointestinal disturbances and weakness. Treatment should be symptomatic and supportive.
