Description
Overview of the medicine
Ficef 500 mg capsule contains Cefixime, a third-generation cephalosporin antibiotic. It is effective against a wide range of gram-positive and gram-negative bacteria, used for treating various bacterial infections.
Uses & Indications
Dosage
Adults
Standard adult dose is 500 mg once daily, or as directed by physician for specific indications. Duration of treatment depends on the infection type and severity, typically 7-14 days.
Elderly
No specific dosage adjustment is required for the elderly unless there is significant renal impairment. Monitor renal function.
Renal_impairment
For creatinine clearance 21-60 mL/min, dose reduction may be considered. For creatinine clearance <20 mL/min, a lower dose (e.g., 250 mg once daily) or extended dosing interval is necessary.
How to Take
Ficef 500 mg capsule should be taken orally with water, with or without food. For best absorption and to minimize gastrointestinal upset, it can be taken with food.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Antimicrobial effects typically begin within hours of administration.
Excretion
Excreted primarily via renal and biliary routes, with a significant portion eliminated unchanged in urine.
Half life
Plasma elimination half-life is usually 3-4 hours, potentially longer in renal impairment.
Absorption
Approximately 40-50% orally absorbed. Peak plasma concentrations are achieved in 2-6 hours after administration.
Metabolism
Undergoes limited metabolism; approximately 50% is excreted unchanged in urine.
Side Effects
Contraindications
- Hypersensitivity to cefixime or any other cephalosporin antibiotics
- History of severe hypersensitivity reaction to penicillin (due to potential cross-reactivity)
Drug Interactions
Carbamazepine
Cefixime may increase carbamazepine plasma concentrations, requiring monitoring of carbamazepine levels.
Warfarin and other oral anticoagulants
Concurrent use may increase prothrombin time (PT) and INR, increasing bleeding risk. Close monitoring of coagulation parameters is recommended.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote. Treatment should be symptomatic and supportive. Hemodialysis or peritoneal dialysis does not remove significant quantities of cefixime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefixime is excreted in small amounts in human milk; use with caution in nursing mothers.

- Complete Blood Count (CBC) with differential, especially during prolonged therapy
- Liver function tests (LFTs) if treatment is extended
Doctor Notes
- Confirm bacterial susceptibility before initiating therapy, if possible.
