Description
Overview of the medicine
Flucopen 125 mg Suspension is an antibiotic used to treat various bacterial infections, primarily those caused by Staphylococcus bacteria. It belongs to the penicillin group of antibiotics and is effective against beta-lactamase producing bacteria.
Uses & Indications
Dosage
Adults
Not typically used for adults in suspension form; usually 250-500 mg every 6 hours (oral capsule/tablet).
Elderly
No specific dosage adjustment for elderly unless renal impairment.
Children
1-5 years: 125 mg 4 times daily; 5-10 years: 250 mg 4 times daily. Dosage may vary based on infection severity and body weight.
Renal_impairment
Dosage reduction may be necessary for severe renal impairment (creatinine clearance <10 ml/min).
How to Take
Take orally, preferably on an empty stomach (30-60 minutes before food or 2 hours after food) to maximize absorption. Shake the bottle well before each use. Use the provided measuring spoon/cup for accurate dosing.
Mechanism of Action
Flucloxacillin is a beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This leads to cell lysis and bacterial death. It is resistant to degradation by beta-lactamase enzymes produced by certain bacteria.
Pharmacokinetics
Onset
Within 1-2 hours
Excretion
Mainly excreted unchanged in the urine via glomerular filtration and tubular secretion.
Half life
Approximately 0.5-1.5 hours
Absorption
Well absorbed from the gastrointestinal tract, though absorption is reduced by food. Peak plasma concentrations achieved within 1-2 hours.
Metabolism
Partially metabolized in the liver, primarily to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to flucloxacillin, other penicillins, or any component of the formulation.
- History of flucloxacillin-associated jaundice or hepatic dysfunction.
- Co-administration with methotrexate.
Drug Interactions
Warfarin
May alter anticoagulant effect; monitor INR.
Methotrexate
Increased methotrexate toxicity due to reduced renal excretion.
Oral Contraceptives
May reduce efficacy of oral contraceptives (though clinical significance is debated, advise additional contraception).
Storage
Store below 30°C in a dry place, protected from light. Keep out of reach of children. After reconstitution, store in a refrigerator (2-8°C) and discard after recommended period.
Overdose
Symptoms may include nausea, vomiting, and diarrhea. Management is symptomatic and supportive. No specific antidote. Haemodialysis is not effective.
Pregnancy & Lactation
Pregnancy: Category B. Generally considered safe, but use only if clearly needed. Lactation: Excreted in breast milk; use with caution. Monitor infants for diarrhea or candidiasis.

- Renal function tests (for patients with renal impairment).
- Complete blood count (rarely, with long-term therapy).
Doctor Notes
- Advise parents on proper measurement and administration techniques for suspension.
- Emphasize completion of the full antibiotic course to prevent resistance.
- Monitor for signs of hypersensitivity, especially with the first dose.
- Be aware of potential liver toxicity, particularly in prolonged or high-dose therapy.
Patient Guidelines
- Complete the full course of treatment as prescribed, even if symptoms improve.
- Shake the bottle well before each use.
- Take on an empty stomach for best absorption.
- Store in a cool, dry place away from direct sunlight.
Missed Dose Advice
