Uses & Indications
Dosage
Adults
GERD/Erosive Esophagitis: 40 mg once daily for up to 8 weeks. Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted as needed. H. pylori eradication: 40 mg twice daily for 7-14 days with appropriate antibiotics.
Elderly
No specific dosage adjustment usually required, but monitor for adverse effects.
Renal_impairment
No dosage adjustment needed for renal impairment. Limited data in severe impairment.
How to Take
Take Fostat 40 mg Tablet once daily before a meal, preferably in the morning. Swallow the tablet whole; do not chew, crush, or split.
Mechanism of Action
Pantoprazole selectively inhibits the H+/K+-ATPase enzyme (proton pump) in the gastric parietal cells, blocking the final step of acid production.
Pharmacokinetics
Onset
Within 2.5 hours, with full acid suppression achieved after several days of therapy.
Excretion
Mainly renal (approximately 80%) and biliary/fecal (approximately 18%) as metabolites.
Half life
Approximately 1 hour (terminal elimination half-life), but acid suppression lasts much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after oral administration; bioavailability approximately 77%. Peak plasma concentrations within 2-2.5 hours.
Metabolism
Extensively metabolized in the liver via CYP2C19 and CYP3A4 enzymes, primarily by demethylation followed by sulfation.
Side Effects
Contraindications
- Hypersensitivity to Pantoprazole, substituted benzimidazoles, or any component of the formulation
- Concomitant use with rilpivirine-containing products due to potential reduction in rilpivirine absorption
Drug Interactions
Warfarin
Possible increased INR and prothrombin time; monitor closely.
Tacrolimus
Increased tacrolimus exposure; monitor tacrolimus concentrations.
Clopidogrel
Reduced antiplatelet effect of clopidogrel; consider alternative antiplatelet therapy.
Methotrexate
Possible increased methotrexate levels, especially with high-dose methotrexate; consider temporary PPI discontinuation.

Overdose
Limited experience with overdose. No specific antidote. Symptomatic and supportive treatment is recommended. Pantoprazole is not readily dialyzable.
