Description
Overview of the medicine
Calcium gluconate injection is used to treat conditions caused by low calcium levels in the blood, such as acute hypocalcemia, magnesium toxicity, and to help stabilize the heart in cases of hyperkalemia.
Uses & Indications
Dosage
Adults
Typical initial dose: 1-2 g (10-20 mL of 10% solution) IV slow push over 5-10 minutes. May be repeated every 6 hours or as continuous infusion as needed. Max 3g per 6 hours.
Elderly
No specific dosage adjustment, but administer with caution due to potential decreased renal function and comorbidities. Monitor calcium levels closely.
Renal_impairment
Use with caution. Dosage adjustments may be necessary, and close monitoring of serum calcium and renal function is critical.
How to Take
Administer intravenously (IV) as a slow injection or diluted in an infusion. Rapid IV injection can cause serious cardiac adverse effects.
Mechanism of Action
Calcium gluconate provides an immediate source of calcium ions, which are essential for nerve, muscle, and cardiac function, bone formation, and blood coagulation. It acts to replenish extracellular and intracellular calcium stores.
Pharmacokinetics
Onset
Rapid, within minutes of IV administration.
Excretion
Primarily renally excreted (glomerular filtration), with some excretion via feces.
Half life
Plasma half-life is approximately 25-50 minutes, but calcium is rapidly distributed to tissues.
Absorption
Rapidly available after intravenous (IV) administration.
Metabolism
Calcium is not metabolized in the traditional sense; it is incorporated into bone or excreted.
Side Effects
Contraindications
- Hypercalcemia
- Ventricular fibrillation
- Concomitant use with IV Ceftriaxone in neonates (due to risk of precipitation)
- Digitalis toxicity
Drug Interactions
Thiazide diuretics
Can decrease calcium excretion and increase the risk of hypercalcemia.
Digitalis (Digoxin)
Increased risk of digitalis toxicity (arrhythmias) if given concomitantly. Administer calcium with extreme caution in digitalized patients.
Phosphates, carbonates, bicarbonates
Incompatible in solution; may cause precipitation.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose are typically signs of hypercalcemia, including nausea, vomiting, constipation, thirst, polyuria, muscle weakness, confusion, and in severe cases, cardiac arrhythmias, coma. Management involves discontinuing calcium, rehydration, and administration of loop diuretics.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Calcium is excreted in breast milk; use with caution in breastfeeding mothers.

WHO Essential Medicine
YesClinical Trials
Calcium gluconate is a long-established essential medicine, and extensive clinical trials for its primary indications have been conducted over decades. Recent trials generally focus on specific populations or novel administration methods.
Lab Monitoring
- Serum calcium levels (before, during, and after treatment)
- ECG monitoring (especially with rapid administration or in digitalized patients)
- Renal function (for prolonged therapy or in patients with kidney impairment)
- Serum magnesium and phosphate levels
Doctor Notes
- Always administer slowly to minimize the risk of cardiac arrhythmias and hypotension.
- Close monitoring of ECG and serum calcium levels is crucial, especially in patients receiving digitalis or with renal impairment.
- Ensure patency of IV access to prevent extravasation and local tissue damage.
Patient Guidelines
- This medication is administered by a healthcare professional in a medical setting.
- Inform your doctor or nurse if you experience any pain, swelling, or redness at the injection site.
- Report any unusual symptoms such as nausea, dizziness, or changes in heart rate immediately.
Missed Dose Advice
Calcium gluconate injection is typically administered in a hospital or clinical setting by a healthcare professional, so missed doses are unlikely. If a dose is missed in an emergency, it will be administered as soon as feasible.
Driving Precautions
Since this injection is typically given in acute or emergency situations and may cause dizziness, patients should avoid driving or operating heavy machinery until they feel well and alert.
Lifestyle Advice
- No specific lifestyle advice directly related to acute intravenous calcium gluconate administration.
