Imbac-500 mg injection is a broad-spectrum antibiotic used to treat a wide range of severe bacterial infections. It contains Imipenem, a carbapenem antibiotic, and Cilastatin, an enzyme inhibitor, which together provide potent bactericidal activity.
Uses & Indications
Dosage
Adults
Usually 500 mg Imipenem (with 500 mg Cilastatin) every 6-8 hours by intravenous infusion. Max daily dose is 4 g Imipenem (with 4 g Cilastatin). Dosage varies based on severity of infection and susceptibility of causative organisms.
Elderly
Dosage should be adjusted based on renal function, similar to adults with renal impairment.
Renal_impairment
Significant dose reduction and/or increased dosing interval required based on creatinine clearance. Consult specific dosing tables.
How to Take
Administer by intravenous infusion over 20 to 60 minutes. The reconstituted solution must be further diluted before infusion. Do not administer by intramuscular or bolus injection.
Mechanism of Action
Imipenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. Cilastatin is a dehydropeptidase I inhibitor that prevents the metabolism of imipenem in the kidneys, ensuring higher and more prolonged concentrations of active imipenem.

Probenecid

Ganciclovir

Valproic acid
May decrease valproic acid plasma concentrations, potentially leading to subtherapeutic levels and increased risk of seizures.
Storage
Store intact vials at controlled room temperature (20-25°C), protected from light. Do not freeze. Reconstituted solutions should be used immediately or stored as per specific manufacturer guidelines.
Overdose
In the event of an overdose, symptomatic and supportive treatment should be initiated. Hemodialysis may be useful for removing imipenem and cilastatin from the circulation.
