Description
Overview of the medicine
Ketorolac Tromethamine is a non-steroidal anti-inflammatory drug (NSAID) used as an ophthalmic solution to reduce inflammation and pain in the eyes, particularly after cataract surgery and for seasonal allergic conjunctivitis. This 0.025% strength is a specific formulation for ophthalmic use.
Uses & Indications
Dosage
Adults
For post-operative inflammation: One drop into the affected eye(s) four times daily, starting 24 hours after surgery and continuing for up to two weeks. For seasonal allergic conjunctivitis: One drop into the affected eye(s) four times daily for temporary relief.
Elderly
No specific dosage adjustment is generally required for elderly patients.
Renal_impairment
No specific dosage adjustment required for ophthalmic use due to minimal systemic absorption.
How to Take
Wash hands thoroughly before use. Tilt head back, pull down the lower eyelid to form a pocket. Squeeze one drop into the pocket without touching the eye or eyelid with the dropper tip. Close the eye gently for a moment. If using other eye drops, wait at least 5 minutes between applications.
Mechanism of Action
Ketorolac acts by inhibiting prostaglandin synthesis through the inhibition of cyclooxygenase (COX-1 and COX-2) enzymes. Prostaglandins are mediators of inflammation and pain. By reducing prostaglandin levels, ketorolac effectively diminishes ocular inflammation and pain.
Pharmacokinetics
Onset
Ocular analgesic effect typically observed within 1 hour.
Excretion
Excreted primarily in urine (for systemically absorbed drug) and feces.
Half life
Systemic half-life is approximately 5-7 hours, but local ocular effects are more clinically relevant.
Absorption
Following topical ocular administration, systemic absorption is minimal. Peak plasma concentrations are very low, typically below detectable limits, indicating primarily local action.
Metabolism
Systemically absorbed ketorolac undergoes hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to ketorolac tromethamine or any component of the formulation, or to other NSAIDs.
- Patients with a history of bleeding tendencies or coagulation disorders.
- Concurrent use with other topical NSAIDs.
Drug Interactions
Systemic NSAIDs
Although systemic absorption is minimal, caution is advised if used concurrently with systemic NSAIDs or anticoagulants, especially in patients prone to bleeding.
Other Ophthalmic Medications
If more than one topical ophthalmic medication is being used, they should be administered at least 5 minutes apart.
Storage
Store below 30°C in a dry place, protected from light. Do not freeze. Keep out of reach of children.
Overdose
Ophthalmic overdose is unlikely to lead to systemic toxicity due to minimal absorption. If excessive drops are accidentally applied, the eye may be flushed with lukewarm water.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether ophthalmic ketorolac is excreted in human milk; caution should be exercised when administered to a nursing mother.

Clinical trials have demonstrated the efficacy and safety of Ketorolac Tromethamine ophthalmic solution in reducing ocular inflammation and pain following various ophthalmic surgeries and in managing seasonal allergic conjunctivitis.
Lab Monitoring
- No specific laboratory monitoring is typically required for ophthalmic ketorolac due to minimal systemic absorption.
Doctor Notes
- Emphasize short-term use, especially in post-operative settings.
- Counsel patients on proper administration technique to prevent contamination and maximize efficacy.
