Description
Overview of the medicine
Labetalol is a combined alpha- and beta-adrenoceptor blocker used to treat hypertension and angina. It effectively lowers blood pressure by reducing peripheral vascular resistance and heart rate.
Uses & Indications
Dosage
Adults
Initial dose 100 mg twice daily; usual maintenance 200-400 mg twice daily. Max 2400 mg/day.
Elderly
Start with lower doses (e.g., 50 mg twice daily) and titrate slowly due to potential for increased sensitivity and co-morbidities.
Renal_impairment
No significant dose adjustment needed for renal impairment, but caution advised.
How to Take
Oral tablets should be taken with food, preferably after a meal, to enhance bioavailability and reduce orthostatic effects. Do not crush or chew.
Mechanism of Action
Labetalol is a non-selective beta-adrenergic blocker and a selective alpha1-adrenergic blocker. It competitively blocks beta1 and beta2 receptors and also blocks alpha1 receptors, leading to vasodilation and reduced heart rate and contractility, thus lowering blood pressure.
Pharmacokinetics
Onset
Oral: 20 minutes - 2 hours; IV: 2-5 minutes
Excretion
Primarily renal (approx. 55-60%) as metabolites and unchanged drug; some via feces.
Half life
Approximately 6-8 hours
Absorption
Well absorbed from the GI tract (approx. 90%), but undergoes extensive first-pass metabolism, resulting in approximately 25% oral bioavailability.
Metabolism
Extensively metabolized in the liver, primarily via conjugation and glucuronidation.
Side Effects
Contraindications
- Bronchial asthma or history of obstructive airway disease
- Heart failure (decompensated)
- Second or third-degree AV block
- Cardiogenic shock
- Severe bradycardia
- Hypotension
Drug Interactions
Cimetidine
Increases Labetalol bioavailability.
General anesthetics
May potentiate hypotensive effects.
Tricyclic antidepressants
May increase risk of tremor.
Non-steroidal anti-inflammatory drugs (NSAIDs)
May reduce antihypertensive effect.
Calcium channel blockers (e.g., verapamil, diltiazem)
Increased risk of bradycardia and heart block.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe hypotension, bradycardia, heart failure, bronchospasm. Management involves supportive measures, IV fluids, atropine for bradycardia, glucagon, and potentially vasopressors.
Pregnancy & Lactation
Pregnancy category C. Use during pregnancy only if potential benefit justifies potential risk to the fetus. Excreted in breast milk; caution advised during lactation. Consult doctor.
