Uses & Indications
Dosage
Adults
The usual dose is 600 mg once daily, administered orally. Duration of treatment depends on the type and severity of infection, typically 7 to 14 days.
Elderly
No specific dose adjustment required based solely on age, but renal function should be assessed, and dose adjusted if needed.
Renal_impairment
Dose reduction is necessary for patients with creatinine clearance less than 50 mL/min. Consult a physician for specific dosage adjustments.
How to Take
Lefabac 600 mg Tablet can be taken with or without food. Swallow the tablet whole with a sufficient amount of water. Do not crush or chew the tablet.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV enzymes, which are essential for bacterial DNA replication, transcription, repair, and recombination. By inhibiting these enzymes, it leads to bacterial cell death.

Onset of action varies depending on the type and severity of infection, typically within a few hours of the first dose.
Excretion
Primarily excreted unchanged in the urine, mainly by glomerular filtration and tubular secretion.
Half life
The elimination half-life is approximately 6 to 8 hours.
Absorption
Rapid and almost complete oral absorption. Bioavailability is approximately 99%. Peak plasma concentrations are reached within 1 to 2 hours after oral administration.
Metabolism
Minimally metabolized in the liver; less than 5% of the dose is metabolized to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to levofloxacin, other quinolones, or any excipients of the product.
- Patients with a history of tendon disorders related to fluoroquinolone administration.
- Epilepsy.
- Children or adolescents below 18 years of age.
- Pregnancy and breastfeeding.
Drug Interactions
Warfarin
May enhance the anticoagulant effects of warfarin. Close monitoring of INR (International Normalized Ratio) is recommended.
Corticosteroids
Concomitant use with corticosteroids may increase the risk of tendon rupture.
Antidiabetic Agents
May cause dysglycemia (hypoglycemia or hyperglycemia), especially in diabetic patients. Monitor blood glucose levels.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May increase the risk of CNS stimulation and convulsive seizures when co-administered with fluoroquinolones.
Antacids (magnesium/aluminum), Sucralfate, Metal Cations (iron, zinc)
Reduce oral absorption of levofloxacin. Administer levofloxacin at least 2 hours before or 2 hours after these agents.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In the event of an overdose, gastric lavage may be considered. Symptomatic and supportive treatment should be initiated. Hemodialysis or peritoneal dialysis is not effective in removing levofloxacin from the body.
Pregnancy & Lactation
Levofloxacin is classified as Pregnancy Category C. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Levofloxacin is excreted into breast milk; therefore, breastfeeding is generally not recommended during treatment.
