Overview of the medicine
Liptor 40 mg tablet contains Atorvastatin, a statin medication used to lower high cholesterol and triglycerides in the blood. It helps reduce the risk of heart attack, stroke, and other heart-related complications.
Uses & Indications
Dosage
Adults
The usual starting dose is 10-20 mg once daily. The dose can be adjusted based on lipid levels and therapeutic goal, up to a maximum of 80 mg once daily. 40 mg is a common therapeutic dose.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
Hepatic_impairment
Liptor is contraindicated in patients with active liver disease or unexplained persistent elevations of serum transaminases. Use with caution in patients with a history of liver disease or alcohol intake.
How to Take
Take Liptor once daily, at any time of day, with or without food. It is recommended to take it at the same time each day to maintain consistent blood levels.
Mechanism of Action
Atorvastatin selectively inhibits HMG-CoA reductase, an enzyme that catalyzes the rate-limiting step in cholesterol biosynthesis. This action reduces cholesterol production in the liver and increases the number of LDL receptors on the liver cell surfaces, leading to increased removal and catabolism of LDL-C from the bloodstream.
Pharmacokinetics
Onset
Lipid-lowering effects typically observed within 2 weeks, maximal by 4 weeks.
Excretion
Primarily excreted in the bile following hepatic and/or extra-hepatic metabolism. Less than 2% of a dose is recovered in urine.
Half life
Plasma elimination half-life is approximately 14 hours. The half-life of HMG-CoA reductase inhibitory activity is 20-30 hours due to active metabolites.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations (Cmax) achieved within 1-2 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 3A4 (CYP3A4) to active ortho- and parahydroxylated metabolites and inactive beta-oxidation products.
Side Effects
Contraindications
- Active liver disease or unexplained persistent elevations of serum transaminases.
- Pregnancy and breastfeeding.
- Hypersensitivity to atorvastatin or any component of the formulation.
Drug Interactions
Warfarin
Atorvastatin may enhance the anticoagulant effect of warfarin; INR should be monitored.
Oral Contraceptives
May increase plasma concentrations of norethindrone and ethinyl estradiol.
Cyclosporine, Gemfibrozil, other Fibrates, Niacin
Concomitant use with these drugs may increase the risk of myopathy/rhabdomyolysis.
Strong CYP3A4 inhibitors (e.g., clarithromycin, itraconazole, protease inhibitors)
May significantly increase atorvastatin plasma concentrations, increasing the risk of myopathy/rhabdomyolysis.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
There is no specific treatment for Liptor overdose. In the event of an overdose, treatment should be symptomatic and supportive. Due to extensive protein binding, hemodialysis is not expected to significantly enhance atorvastatin clearance.
Pregnancy & Lactation
Liptor is contraindicated during pregnancy (Pregnancy Category X) due to the potential for fetal harm. Women of childbearing age should use effective contraception. It is also contraindicated during breastfeeding.
