Nitrofurantoin is an antibiotic used to treat and prevent urinary tract infections (UTIs). It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Acute UTI: 50-100 mg four times daily for 7 days. Prophylaxis: 50-100 mg once daily at bedtime.
Elderly
Use with caution; monitor renal function. Dosage adjustment may be needed if creatinine clearance is below 60 mL/min.
Renal_impairment
Contraindicated if creatinine clearance is <60 mL/min.
How to Take
Take with food or milk to enhance absorption and minimize gastrointestinal upset. Swallow whole with water.
Mechanism of Action
Nitrofurantoin is reduced by bacterial flavoproteins to reactive intermediates that inactivate or damage bacterial ribosomal proteins, DNA, RNA, and other macromolecules.

Drug Interactions
Quinolone antibiotics
May have antagonistic effects.
Probenecid, Sulfinpyrazone
May decrease renal excretion of nitrofurantoin, increasing plasma levels and toxicity, while decreasing its effectiveness in the urine.
Magnesium trisilicate antacids
May reduce the absorption of nitrofurantoin.
Storage
Store at room temperature (below 30°C), protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, and diarrhea. Management is supportive; induce emesis or gastric lavage if recent ingestion. Maintain adequate hydration. Hemodialysis is ineffective.
Pregnancy & Lactation
Pregnancy: Category B. Avoid in the third trimester due to risk of hemolytic anemia in the newborn. Use with caution in other trimesters if benefits outweigh risks.
