Description
Overview of the medicine
Afexa 120 mg Tablet contains Fexofenadine Hydrochloride, a non-sedating antihistamine used to relieve symptoms of seasonal allergic rhinitis and chronic idiopathic urticaria. It works by blocking histamine H1 receptors, thereby reducing allergic reactions like sneezing, runny nose, itchy eyes, and skin rashes.
Uses & Indications
Dosage
Adults
Seasonal Allergic Rhinitis: 120 mg once daily. Chronic Idiopathic Urticaria: 180 mg once daily (if available, otherwise 120 mg once daily for rhinitis symptoms).
Elderly
No specific dosage adjustment is generally required, but caution is advised in patients with impaired renal or hepatic function.
Renal_impairment
For patients with moderate to severe renal impairment (creatinine clearance < 80 mL/min), initial dose of 60 mg once daily is recommended. Use with caution.
How to Take
Take the tablet orally with water, preferably before a meal. Do not take with fruit juice.
Mechanism of Action
Fexofenadine is a selective peripheral H1-receptor antagonist. It binds to histamine H1 receptors in the body, blocking the action of histamine, which is a key mediator of allergic reactions. This leads to a reduction in allergy symptoms without significant sedation, as it does not readily cross the blood-brain barrier.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Primarily excreted in feces (approximately 80%) and urine (approximately 11%) as unchanged drug.
Half life
Approximately 11-15 hours
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached in 1-3 hours.
Metabolism
Minimally metabolized; approximately 5% of the total dose is metabolized (mainly hepatic).
Side Effects
Contraindications
- Hypersensitivity to Fexofenadine Hydrochloride or any component of the formulation.
Drug Interactions
Erythromycin or Ketoconazole
Can increase plasma levels of fexofenadine, potentially leading to increased side effects, though generally not clinically significant.
Antacids (aluminum and magnesium containing)
May reduce the bioavailability of fexofenadine. Separate administration by at least 2 hours.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, common symptoms may include dizziness, drowsiness, and dry mouth. Treatment should be symptomatic and supportive. Hemodialysis does not effectively remove fexofenadine from blood.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Fexofenadine is excreted in human breast milk; use with caution in nursing mothers.
