Description
Overview of the medicine
Breasy 5 mg Tablet contains Montelukast, a leukotriene receptor antagonist, used for the prophylaxis and chronic treatment of asthma and for the relief of symptoms of seasonal allergic rhinitis.
Uses & Indications
Dosage
Adults
One 5 mg tablet daily in the evening for asthma and/or allergic rhinitis, especially if prescribed by a doctor (10 mg is more common for adults). For exercise-induced bronchoconstriction, one 5 mg tablet at least 2 hours before exercise, not more than once daily, if prescribed.
Elderly
No dosage adjustment needed.
Children
For children 6-14 years: One 5 mg tablet daily in the evening. For children 2-5 years: One 4 mg chewable tablet daily in the evening (if 5mg tablet is split or chewable form is not available, consult a doctor).
Renal_impairment
No dosage adjustment needed.
How to Take
Take orally, once daily in the evening, with or without food. For exercise-induced bronchoconstriction, take at least 2 hours before exercise. Do not take an additional dose for exercise-induced bronchoconstriction if you are already taking a daily dose.
Mechanism of Action
Montelukast selectively blocks the cysteinyl leukotriene 1 (CysLT1) receptor. Leukotrienes are potent inflammatory mediators released from various cells, including mast cells and eosinophils, that contribute to bronchoconstriction, mucus secretion, vascular permeability, and eosinophil recruitment in asthma and allergic rhinitis.

Excretion
Primarily excreted in faeces via bile (about 86%), with less than 0.2% excreted in urine.
Half life
Approximately 2.7 to 5.5 hours
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration achieved in 2-4 hours for adults.
Metabolism
Extensively metabolized by cytochrome P450 enzymes (CYP3A4, CYP2C9, CYP2A6) in the liver.
Side Effects
Contraindications
- Hypersensitivity to Montelukast or any component of the formulation.
- Acute asthma attacks (Montelukast is not indicated for the reversal of bronchospasm in acute asthma attacks).
Drug Interactions
Rifampicin
May decrease Montelukast plasma concentrations.
Phenobarbital
May decrease Montelukast plasma concentrations.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, the most frequently reported adverse events include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive. It is not known whether Montelukast is dialyzable.
Pregnancy & Lactation
Use during pregnancy only if clearly needed. Montelukast is excreted in breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor.
