Clindamycin is a lincosamide antibiotic used to treat various bacterial infections, including skin and soft tissue infections, respiratory tract infections, and intra-abdominal infections.
Uses & Indications
Dosage
Adults
150-450 mg every 6-8 hours, depending on the severity of the infection. For Clincin 300 mg capsule, typically 300 mg every 6-8 hours.
Elderly
No specific dosage adjustment is generally required based on age alone, but monitor renal and hepatic function.
Renal_impairment
In severe renal impairment (CrCl < 30 mL/min), dosage adjustment may be necessary. Consider extending the dosing interval or reducing the dose. No adjustment needed for mild to moderate impairment.
How to Take
Take orally with a full glass of water, with or without food. To minimize the risk of esophageal irritation, do not lie down for at least 30 minutes after taking the capsule.
Mechanism of Action
Clindamycin binds to the 50S ribosomal subunit of susceptible bacteria, thereby inhibiting bacterial protein synthesis. It typically acts as a bacteriostatic agent.
Pharmacokinetics
Onset
Antibacterial effect typically begins within 30-60 minutes after oral administration.
Excretion
Approximately 10-20% of the dose is excreted in the urine, and about 4% is excreted in the feces, primarily as active drug and metabolites. The remainder is eliminated via biliary excretion.
Half life
The elimination half-life is approximately 2 to 3 hours in adults, which may be slightly prolonged in patients with severe renal or hepatic impairment.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are reached in approximately 45-60 minutes.
Metabolism
Extensively metabolized in the liver to active (e.g., N-demethyl clindamycin) and inactive metabolites. CYP3A4 and to a lesser extent CYP3A5 are the principal enzymes involved.
Side Effects
Contraindications
- Hypersensitivity to clindamycin, lincomycin, or any component of the formulation.
- History of antibiotic-associated colitis, especially *Clostridioides difficile*-associated diarrhea.
Drug Interactions
Erythromycin
Antagonism between clindamycin and erythromycin has been demonstrated *in vitro*. Avoid co-administration.
Kaolin-pectin
May reduce the absorption of clindamycin. Administer at least 2 hours apart.
Neuromuscular Blocking Agents
Clindamycin has neuromuscular blocking properties and may enhance the action of other neuromuscular blocking agents. Use with caution.
Oral Anticoagulants (e.g., Warfarin)
May increase the effects of vitamin K antagonists. Monitor INR (International Normalized Ratio) closely.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment is recommended. There is no specific antidote for clindamycin, and it is not effectively removed by hemodialysis or peritoneal dialysis.
