Description
Overview of the medicine
Corzil 40 mg Tablet contains Telmisartan, an Angiotensin Receptor Blocker (ARB) used for the treatment of essential hypertension and to reduce cardiovascular morbidity and mortality in patients aged 55 years or older at high risk of cardiovascular events.
Uses & Indications
Dosage
Adults
The usual recommended dose is 40 mg once daily. The dose may be increased to a maximum of 80 mg once daily if necessary.
Elderly
No dose adjustment is necessary for elderly patients.
Renal_impairment
No dose adjustment is required for patients with mild to moderate renal impairment. For severe impairment, close monitoring is recommended.
How to Take
Corzil tablets are for oral use and should be taken once daily with or without food. Swallow the tablet whole with water.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor in many tissues, including vascular smooth muscle and the adrenal gland. This leads to inhibition of the vasoconstrictor and aldosterone-secreting effects of angiotensin II, resulting in vasodilation and reduced blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect usually starts within 3 hours after a single oral dose.
Excretion
Primarily excreted unchanged in feces (about 97%).
Half life
Approximately 24 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Absolute bioavailability varies with dose: approximately 42% for 40 mg and 58% for 160 mg.
Metabolism
Metabolized by conjugation to form an acylglucuronide, which is pharmacologically inactive.
Side Effects
Contraindications
- Hypersensitivity to the active substance or to any of the excipients.
- Second and third trimesters of pregnancy.
- Biliary obstructive disorders.
- Severe hepatic impairment.
- Concomitant use of Aliskiren in patients with diabetes mellitus or renal impairment (GFR < 60 mL/min/1.73 m²).
Drug Interactions
Digoxin
Increased plasma concentrations of digoxin have been reported when co-administered with telmisartan.
Lithium
Concurrent use may lead to reversible increases in serum lithium concentrations and toxicity.
Aliskiren
Contraindicated in patients with diabetes or renal impairment due to increased risk of hypotension, hyperkalemia, and worsening renal function.
Diuretics (Thiazide or loop diuretics)
May increase the risk of symptomatic hypotension.
Non-Steroidal Anti-Inflammatory Drugs (NSAIDs)
May reduce the antihypertensive effect of Telmisartan and increase the risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited data are available concerning overdose in humans. The most likely manifestations would be hypotension, tachycardia, or bradycardia. Treatment is symptomatic and supportive. Telmisartan is not removed by hemodialysis.
Pregnancy & Lactation
Telmisartan is contraindicated during the second and third trimesters of pregnancy due to the risk of fetal harm. It is not recommended during the first trimester and during lactation.

Lab Monitoring
- Serum potassium levels (especially in patients with renal impairment or those taking potassium-sparing diuretics).
- Renal function (creatinine, GFR) periodically.
Doctor Notes
- Advise patients on the importance of consistent medication adherence for optimal blood pressure control.
- Monitor renal function and serum potassium levels regularly, especially in high-risk groups.
- Educate patients about potential symptoms of hypotension and hyperkalemia.
Patient Guidelines
- Take Corzil exactly as prescribed by your doctor, even if you feel well.
- Do not stop taking Corzil abruptly without consulting your doctor, as this may worsen your condition.
- Regularly monitor your blood pressure at home if advised by your doctor.
