Etoricoxib is a non-steroidal anti-inflammatory drug (NSAID) specifically a COX-2 selective inhibitor. It is used for the treatment of various inflammatory and painful conditions.
Uses & Indications
Dosage
Adults
For Osteoarthritis: 60 mg once daily. For Rheumatoid Arthritis, Ankylosing Spondylitis: 90 mg once daily. For Acute Gouty Arthritis: 120 mg once daily for a maximum of 8 days. For Acute Pain, Primary Dysmenorrhea: 120 mg once daily for a maximum of 8 days.
Elderly
No dose adjustment required, but caution is advised. Initiate at the lowest effective dose.
Renal_impairment
Mild to moderate impairment: No dose adjustment. Severe impairment (CrCl <30 mL/min): Not recommended.
How to Take
Orally, with or without food. For faster onset, it can be taken without food.
Mechanism of Action
Etoricoxib selectively inhibits cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins, mediators of pain, inflammation, and fever.

Contraindications
- Hypersensitivity to etoricoxib or any component of the formulation

- Congestive heart failure (NYHA II-IV)
- Established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Drug Interactions
Lithium
Increased plasma lithium levels, leading to toxicity.
Warfarin
Increased risk of bleeding.
Methotrexate
Increased plasma methotrexate levels, leading to toxicity.
ACE inhibitors/ARBs
Reduced antihypertensive effect and increased risk of renal impairment.
Oral Contraceptives
Increased plasma concentration of oral contraceptives.

Overdose
Symptoms include gastrointestinal disturbances (e.g., nausea, vomiting, abdominal pain), somnolence. Management is supportive; there is no specific antidote. Gastric decontamination (e.g., activated charcoal) may be considered within an hour of ingestion.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation. NSAIDs, including etoricoxib, may cause premature closure of the fetal ductus arteriosus if used in the third trimester of pregnancy.
