Overview of the medicine
Lansoprazole is a proton pump inhibitor (PPI) used to treat and prevent various conditions involving excess stomach acid, such as stomach ulcers, gastroesophageal reflux disease (GERD), and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD: 30 mg once daily for up to 8 weeks. For duodenal ulcer: 15 mg once daily for 4 weeks. For gastric ulcer: 30 mg once daily for 8 weeks. For H. pylori eradication: 30 mg twice daily (in combination therapy).
Elderly
No specific dose adjustment is generally required, but monitor for adverse effects.
Renal_impairment
No dose adjustment is generally required.
Hepatic_impairment
Dose reduction may be considered in severe hepatic impairment (e.g., maximum 15 mg daily).
How to Take
Take orally, usually once daily before a meal (e.g., 30 minutes before breakfast). Swallow the capsule whole; do not chew or crush. If difficulty swallowing, capsules may be opened and granules mixed with a small amount of applesauce or juice.
Mechanism of Action
Lansoprazole works by irreversibly binding to and blocking the H+/K+-ATPase enzyme (the proton pump) in the gastric parietal cells. This action effectively reduces the secretion of gastric acid, leading to a decrease in stomach acid levels.

Onset
Acid suppression begins within 1-2 hours.
Excretion
Approximately 1/3 is excreted in urine and 2/3 in feces as inactive metabolites.
Half life
Plasma elimination half-life is 1.5-2 hours, but its pharmacodynamic effect is much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration. Bioavailability is high (around 80-90%). Peak plasma concentrations are achieved within 1.5-2 hours.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 enzyme system, primarily by CYP2C19 and CYP3A4.

Contraindications
- Known hypersensitivity to lansoprazole or any excipients in the formulation.
- Co-administration with atazanavir or nelfinavir due to significant reduction in their bioavailability.

Digoxin
Increased absorption of digoxin due to increased gastric pH.
Warfarin
Possible increase in INR and prothrombin time, requiring monitoring.
Tacrolimus
Increased plasma concentrations of tacrolimus, requiring monitoring.
Clopidogrel

Methotrexate

Overdose
There is no specific antidote for lansoprazole overdose. Treatment should be symptomatic and supportive. Lansoprazole is not significantly removed by hemodialysis.
